Transition Professionals assists people released from jail or prison via a gateway to services and support needed upon reentry to the community.
Since 2014, our team has been dedicated to improving the lives of individuals transitioning from jail back into society. Over the past decade, we’ve had the privilege of working with over 1,700 individuals, primarily in Northern New Jersey, providing them with essential resources, counseling, and direct support. Every year, we assist more than 300 men and women in rebuilding and transforming their lives.
